Chronic Disease Prevention and Management

Chronic Disease Prevention and Management (CDPM) in General Practice is accepted as a priority in Australia where over three million people suffer daily from chronic disease, that's nearly one in seven people that attend your practice.

Chronic Diseases such as Diabetes, Cancer, Cardiovascular Disease, Asthma and other long term health conditions are considered to be the most significant contributors to morbidity and mortality in Australia.

General Practitioners are commonly the first point of contact for many people with chronic disease and have a vital role in early identification, prevention, diagnosis and management of chronic disease in the community.
GP down south can assist with;
• education and training for GPs, nurses, general practice staff and other health providers within the community
• promoting, developing and maintaining partnerships/relationships between general practice, other service providers and the community at a local level to improve access to quality, timely and coordinated care.
